Please inform us if you are currently experiencing a fever, cough, or difficulty breathing – your appointment will need to be rescheduled. Please inform us of any recent travel outside or within the United States. We ask that you have all other forms completed and up-to-date prior to your appointment.
To reduce the number of people in the waiting room, please arrive at your appointment time. We ask that you enter wearing a face mask. We will no longer be holding personal items in the front office during visits (coats, backpacks, etc.).
Upon your arrival at our office, we kindly ask that you use the hand sanitizer we have provided. Please disinfect your hands prior to using iPads to fill out any documents we may need from you. We recommend you bring your own smartphone or other device to fill out any of these documents. If using the restroom or exiting and reentering our office, we ask that you use the hand sanitizer provided upon reentry.
We will be wiping down and properly disinfecting (with EPA-registered disinfectants) countertops, door handles, chairs, pens, bathrooms, etc. routinely throughout the day. We have removed items from our office that are touched by multiple people and may not be easily disinfected, such as magazines, pamphlets, and books.
Please note that we will be limiting social distance, including handshaking, as a means of decreasing possible transmission of coronavirus (COVID-19). No one will be permitted into the office aside from the person scheduled for an appointment. Please have any companions wait outside or stay home during this time.
We will ONLY be taking contactless credit card payments as a means of decreasing possible transmission of coronavirus (COVID-19). We will be requiring an arrangement to leave a card on file to help make this transition possible.
Do I have to get a virus test before my dental appointment? UPDATED 9/2021
Testing is not required if you have received both doses of the COVID-19 vaccine.
Testing no longer recommended for any procedure however a screening is required prior to each appointment.
Upon entering your temperature will be taken with a contactless forehead thermometer and we will review the screening questions with you.
We will be wearing face shields , gowns, and N-95 masks for everyone’s protection. We have spent countless hours responsibly sourcing our PPE (personal protective equipment) to provide you with the best and safest experience.
All those who enter the office will be required to wear a face mask and for it to be left on until instructed otherwise.
We will be limiting entrance into the office to maintain the social distancing requirement of 6 feet.
Our staff will continue our practice of proper hand hygiene and washing hands for at least 20 seconds before putting on gloves.
You will be asked to rinse with a 1.5% hydrogen peroxide solution for 1 minute as a pre-procedural mouth rinse.
We will strictly require use of an ISOLATE type device if an aerosol-producing procedure is being performed to help minimize aerosol or spatter.
We have air purifiers in each room to help with clean air management.
